Rulide is a semisynthetic antibiotic belonging to the group of macrolides. The spectrum is a broad one and comprises sensible strains (Streptococcus pyogenes, mitis, sanguis, viridans, agalactiae, pneumoniae, Bordetella pertussis, Corynebacterium diphtheriae, Listeria monocytogenes, Moraxella catarrhalis, Clostridium, Pasteurella multocida, Neisseria meningitidis, Mycoplasma pneumoniae, Chlamydia trachomatis, psittaci and pneumoniae, Ureaplasma urealyticum, Gardnerella vaginalis, Legionella pneumophila, Helicobacter pylori), strains with variable sensitivity (Staphylococcus aureus and epidermidis, Bacteroides fragilis, Haemophilus influenzae, Vibrio cholerae) and resistant strains (Acinetobacter spp., Enterobacteriaceae, Pseudomonas spp.).

Rulide is more stable in acid environments than other antibiotics. It is present in serum within 15 minutes after administration. 

Common use.

Rulide is used to treat infections sensible to Roxithromycin:

  1. respiratory tract infections- communitary pneumonia, especially pneumonia produced by Mycoplasma and Chlamydia
  2. ENT infections- tonsilitis, pharyngitis and otitis
  3. urogenital infections- mainly those produced by Chlamydia trachomatis
  4. skin and soft tissues infections- recurrent furunculosis, pyoderma (skin inflammation with pus), impetigo, erysipelas.

Administration and dosage.

The regular dose is 150mg of Roxithromycin twice a day. For a better absorption Rulide pills should be swallowed as they are, without crushing them, with a sufficient quantity of water, 15 minutes before meals. The dose recommended for adults is also suitable for children weighing more than 40kg.

Precautions and warnings.

Please be very careful before using this drug if you have hypokalemia, arrythmia or known ECG changes. Obviously, you should consult your doctor first.

If case if you have acute liver failure and taking Rulide is absolutely necessary, then you should take only half of the usual administered dose.

As well, if you have acute diarrhea, you should take into account the possibility of having pseudomembranous colitis. If this is not infirmed, you should stop your treatment with Roxithromycin.

If the period of treatment with Roxithromycin exceeds 14 days, you should pass regular blood tests and laboratory examinations for renal and hepatic functions.

Contraindications:

Do not use Rulide in the following cases:

  1. hipersensitivity to roxithromycin, macrolides or other drug components
  2. you are under treatment with vasoconstrictors, such as ergot alkaloids
  3. if you are taking terfenadine, astemizole, cisapride- this may determine heart rythm disorders and ECG modifications

Rulide Side effects.

As all the drugs Rulide may have side effects.

Frequent:

  1. gastro-intestinal: nausea, abdominal pain

Less frequent:

  1. headaches, vertigo
  2. gastro-intestinal: diarrhea, dyspepsia, vomiting, constipation, flatulence
  3. skin: reddening of the skin, rash (exanthema)

Rare:

  1. swelling of the face, tongue, dyspnea and even anaphylactic shock, eczema, ear noise and taste changes

Drug interaction.

Rulide may interact with:

  1. antihistamine medications
  2. oral contraceptives
  3. digoxin or other glycoside- roxithromycin may increase intestinal absorption of the latter ones
  4. migraine medications
  5. immunosupressants

Pregnancy and breastfeeding: The safety of roxithromycin during pregnancy has not been established and there is no clinical experience regarding roxithromycin’s effect in nursing mothers. Therefore, the use of Roxithromycin by pregnant women and nursing mothers will only be done if being absolutely necessary.

Missed dose.

If you missed one dose of Rulide, take it as soon as you remember. Nevertheless, if it is close to the next dose time, then you should just skip the missed dose. All the next doses should be adminstered as prescribed by the doctor.

Overdose.

The symptoms of an overdose are: nausea, skin rash, lower quantity of urine, seizure. In any of these situations please see your doctor or go to the nearest hospital immediately.
